Consent & Acknowledgement

1. Participation Consent

I confirm that my child is medically fit and able to participate in dance classes. I understand that danceinvolves physical activity and carries an inherent risk of injury.

2. Medical Consent

In the event of an emergency, I authorise Applause Dance Academy staff to seek appropriate medicaltreatment for my child if I cannot be contacted.

3. Liability Waiver

I acknowledge that participation in dance classes is at my own risk. I hereby release, waive and discharge Applause Dance Academy, its instructors and staff from liability for injuries, damages or losses sustained during classes or on the premises, except where caused by negligence.

4. Code of Conduct

I understand that respectful behaviour is expected at all times. Applause Dance Academy reserves the right to refuse admission or terminate participation if behaviour is deemed inappropriate or disruptive.

5. Payments & Refund Policy

I understand and agree to the Academy's fee structure and payment policy. I acknowledge that fees are non-refundable unless otherwise stated within the Academy's policies.

6. Data Protection

I consent to Applause Dance Academy collecting, storing and processing my personal information for administrative purposes in accordance with applicable UK data protection legislation.

Important

Please ensure this form is updated every six months, or sooner if any information changes. Failure to notify Applause Dance Academy of any changes may result in the Academy being unable to respond appropriately in the event of an emergency.
